A bright orange postcard was recently sent home with important information about student health insurance. Major points are:
- Students will AUTOMATICALLY be enrolled in student health insurance for the 2014-2015 school year.
- Every student will see a charge of $1699 on their student account. If you do not want this student health insurance, you must waive it online by providing proof of current health insurance at www.eiiastudent.org/gustavus by August 29th, 2014.
- Once the waiver is complete, every student will see a $1699 credit to their student account within 5 business days.
- This NEW plan has a $250 deductible and a $2500 out of pocket maximum. Visit www.gustavus.edu/studenthealthinsurance for more plan details.
- The deductible will be waived for appointments occurring at the Gustavus Health Service.
- For students who want the NEW plan, no further action is needed, coverage begins August 10th, 2014.
Again, to “opt out” or waive the insurance, please visit this link by August 29th, 2014. If you know you will do this, why not take a few minutes right now and take care of it? You’ll need only the following information:
Student Information (Name, Email Address, DOB, Gustavus student ID Number)
Head of Household Information (First and Last Name, Phone Number)
Insurance Company Information (Name, Phone Number, ID Number)
